A Clinical Study on the Various Laboratory and Radioisotopic Examinations in Hyperthyroid and Euthyroid Patients


Abstract
This study was undertaken to elucidate the influences of thyroid hormon upon various chemical and radioisotopic examinations, particularly in regard to liver function, and to evaluate validity of those tests in hyperthyroidism and euthyroid state.
The results of T-3 and T-4 Kit test, external scintillation counts of radioactivity in the liver after oral administration of Na^(121)I, radioisotope liver function test with colloidal Au-198 and I-131 rose bengal, chemical liver function test and oral glucose tolerance test in 50 patients with hyperthyroidism were compared with those of such various tests in 30 euthyroid patients.
The summarized findings are as follows:
1) The mean value of T-4 test in 50 hyperthyroid cases was 18.1¡¾3.6 rug/dl. while that in 30 euthyroid cases was 8.1¡¾2.4 ¥ìg/dl., and the mean values of T-3 test in 20 hyperthyroid patients was 40.6¡¾10.2% while that in 20 euthyroid patients was 26.2¡¾3.9%. The results of both tests showed higher diagnostic accuracy than those of other conventional methods.
2) External scintillation counts of hepatic radioactivity obtained 24 hours after oral administration of 50 ¥ìg. Na^(131)I was 18,840¡¾6,216 c. p. m. in hyperthyroid patients and 12,776¡¾4,321 c. p. m. in euthyroid patients.
3) Hepatic uptake half time and blood disappearance half time with Au-198 colloid in 30 hyperthyroid patients were 5.87¡¾1.28 and 5.66¡¾1.19 minutes while those in 30 euthyroid patients were 5.43¡¾1.57 and 5.44¡¾1.70 minutes respectively. And hepatic uptake half time and blood disappearance half time with I-131 rose bengal were 10. 16¡¾3.42 and 9.32¡¾2.47 minutes in 30 hyperthyroid patients and 8.24¡¾2.26 and 8.04¡¾2.08 minutes in 30 euthyroid patients. The difference of half times between both groups was found significant only when I-131 rose bengal was used, arid it was not significant in the case that Au-198 was given.
Therefore, it may be taken as that parenchymal liver cells are damaged but reticuloendothelial cells are not affected in hyperthyroidism.
4) The mean values of plasma glucose in fasting state, 30, 60, 120 and 180 minutes after oral administration of 100 grams of glucose were 80.4¡¾9.7, 130.3¡¾18.4, 124.1¡¾21.6, 98.6¡¾13.8 and 83.3¡¾14.3 mg.% in euthyroid patients, while the mean values in hyperthyroid patients were 89.0¡¾14.2, 168.3¡¾21.7, 158.8¡¾19.3, 119.4¡¾16.5 and 91.3¡¾18.2 mg. % respectively. The difference between two groups was most conspicuous 30 and 60 minutes after administration of glucose.
5) Among the various data of conventional liver function test, the significant difference between two, groups was only the value of total cholesterol and its ester.
Total cholesterol and its ester were 193.4¡¾42.3 and 146.8¡¾32.7 mg. % in euthyroid patients, and 140.6¡¾38.6 and 103.1¡¾29.4 mg. % in hyperthyroid patients.
